The insurance forms below are for MLC MasterKey Business Super, MLC MasterKey Personal Super and MLC MasterKey Super Fundamentals members insured under policies issued to the Trustee by the insurer.

Insurance application - short form (PDF)
Form name: MLC MasterKey short form personal statementIf you’re applying for new cover, or prefer not to increase your cover online, you can use the short application form if:
- you’d like to apply for Death-only or Death and Total & Permanent Disablement (TPD) insurance cover (max $1 million total, including any existing insurance) or Income Protection (max $8,000 per month including any existing insurance), and
- you’re able to answer ‘No’ to questions 1-11 in section D of the form.
