    The forms below are for clients insured under policies issued to the Trustee by the named insurer. If you’re unsure which insurer is relevant, please contact us on 132 652 Monday to Friday between 8 am and 6 pm (AEST/AEDT). Alternately, your clients can access their forms by logging into their account at mlc.com.au.

    Insurance application - short form (PDF)
    Form name: MLC MasterKey short form personal statement

    The short application form can be used if your client is applying for:

    • Death-only or Death and Total & Permanent Disablement (TPD) insurance cover (max $1 million total, including any existing insurance) or Income Protection (max $8,000 per month including any existing insurance), and
    • they’re able to answer ‘No’ to questions 1-11 in section D of the form.

    The long application can be used if your client doesn’t meet the short application form conditions above.
